PUBLIC NOTICE

In keeping with current State and Federal regulation and Northwestern Community Services policy governing the retention and destruction of client health records, notice is hereby given of our intent to destroy by shredding the records of those individuals who last received treatment at one of our facilities to include “The Institute” prior to June 1, 1998 and who are not currently active clients.

Destruction of these files is scheduled to begin on or about September 18, 2008.

You may contact the Medical Records Department, Northwestern Community Services, 209 West Criser Road, Suite 300, Front Royal, VA 22630 if you wish to receive a copy of the records of the treatment you received prior to June 01, 1998 and you are currently not being seen at one of the Northwestern Community Services’ facilities. Your written request must be received prior to September 18, 2008.
