REACH PROGRAM

REACH is a lifespan program that serves children and adults across the state.

The REACH program is designed to meet the crisis support needs of individuals who have a developmental disability and are experiencing crisis events that put them at risk for homelessness, incarceration, hospitalization, and/or danger to self or others. REACH programs offer an additional layer of support to emergency services, the state hospital system, and caregivers who work most closely with individuals with a developmental disability (DD).

Who is eligible for REACH services?

Individuals must have a documented intellectual and/or developmental disability AND co-occurring mental illness and/or significant behavioral challenges.

COMMONWEALTH COORDINATED CARE PLUS WAIVER (CCC+)

This waiver is the former Elderly and/or Disabled Consumer Directed (EDCD) Waiver and the Assisted Technology (AT) Waivers. These waivers were combined into one singular waiver which is the Commonwealth Coordinated Care Plus (CCC+) Waiver.

Criteria for admission:

  • Over 65 years old with a medical or nursing need
  • Under 65 years old with a medical or nursing need AND a disability

To request CCC Plus Waiver services, contact your local Virginia Department of Social Services (DSS) or local Virginia Department of Health (VDH).

DD Waiver Information

Community Services Boards (CSBs) are the single point of entry for the statewide DD Waiver waitlist. Individuals who are found eligible are added to the waitlist, however, eligibility does not guarantee immediate access to services. When waiver slots become available, they are assigned based on highest level of need. Wait times vary and may be lengthy. 

How to request a DD Wavier Screening

Please submit the following documentation before scheduling:

  • Birth certificate
  • Social Security card or number
  • Insurance card(s)
  • Guardianship or POA documents (if applicable)
  • ID card (adult) or parent/guardian ID (child)
  • Signed psychological evaluation or IEP (if completed by a school psychologist)
  • Signed medical documentation confirming a DD diagnosis

Submit all documentation to the attention of DD Intake at your local NWCSB clinic (Winchester, Warren, Woodstock, Page) prior to scheduling your screening.
