October 7, 2024 – updated July 7, 2025 – On or around August 8, 2024, Northwestern Community Services Board became aware of unauthorized activity on our network. Upon discovery, we took immediate action to secure the network and engaged a team of third-party specialists to investigate the incident. Our investigation determined that a limited amount of information may have been accessed between July 18, 2024, and August 8, 2024.

Following a thorough analysis, on May 12, 2025, the investigation determined that potentially affected information included patient names, addresses, dates of birth, Social Security numbers, health insurance information, patient medical record numbers, doctor’s name or practice type, medical date of service, medication information, diagnosis and treatment information, doctor names, and medical claims and billing information.

In response to this incident, we partnered with third-party specialists to implement additional security measures within our network and reviewed our policies and procedures related to data protection. Additionally, although we have no evidence of misuse of information as a result of this incident, we encourage potentially impacted individuals to enroll in complimentary credit monitoring and identity protection services we are making available.

Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors.
